Registration Forms
The district registration forms are available in a PDF format for parent(s)/guardian(s) to complete on line. Please download and save each form to your computer in order to enable the fields. All fields in red are required and must be completed. You will need Adobe Acrobat DC installed on your computer to complete the forms.- Student Registration Form

- Residency Information - Please bring the required original documentation to the school site when registering your student.
When registering your new student, please bring the completed forms listed above as well as the following required documents to the school office:- Proof of birthdate/age - please provide one of the following: Birth record, Birth certificate, passport, baptism certificate, or affidavit of the parent, guardian, or custodian of the minor

- Residency Documentation
- There will also be additional school specific forms that will need to be completed at the time of registration.
