PALS Forms and Program Information:

Below are the current renewal and new application forms for the 2018-19 school year.  Please note, if you have an incoming kindergarter, you will need to register your child at your home school between.This applies to all incoming kindergartners, even if there are siblings in the PALS program.  If your child is accepted, then the registration information will be transferred to PALS.
